Supported Living

At Axiom Care, we are dedicated to empowering individuals with disabilities and complex needs to live fulfilling, independent lives. Our person-centered services aim to provide the assistance required while encouraging autonomy and inclusion within the community.

We support children, adolescents and adults(18-65+age groups) with a wide range of conditions including learning disabilities, autism spectrum disorder, Down syndrome, cerebral palsy, spina bifida, epilepsy, mobility impairments, and more. Our trained and compassionate staff are well-versed in each individual's needs and preferences.

Our disability support services include:

- In-home care to assist with personal care, meals, medications, transportation and household tasks. Caregivers become trusted partners promoting dignity and self-sufficiency.

- Community living services for those wanting an independent or semi-independent lifestyle outside the family home. We help find optimal housing, coordinate care services, and provide ongoing support.

- Respite care to give family caregivers a much-needed break. Our caregivers keep routines consistent in their absence so loved ones feel comfortable.

- Social care to facilitate community and social engagement through various activities and outings. Maintaining connections is so important.

- Transition support for adolescents moving into adulthood. We help plan the shift to maximize autonomy.
